RelaiSanté helps hospitals and families find home-care professionals for patients who need support after leaving hospital. Families can compare provider availability and fees, while care teams can coordinate the handoff to services outside the hospital.
The network spans home-care professionals and medical-equipment providers. RelaiSanté reports connections to more than 350 facilities and 10,000 providers, giving the service a base to extend its hospital-to-home workflow.
