Vheda partners with health plans to identify members with complex or chronic needs and tailor outreach to them. Members receive an iPhone and connected devices that capture measures such as blood pressure, weight and blood sugar; its analytics interpret more than 20 data points to guide personalized messaging and program support.

A dedicated care team follows up in real time, coordinates with providers and helps members stay connected to care. Plans use the service for chronic-condition and maternity programs across Medicaid, Medicare and special-needs populations. The company reports operating these programs for more than a decade and is expanding nationally.
